Flying Start Advocates and its associated health professionals collect personal and medical information.
The clinic and its associated health professionals may communicate with my family doctor and/or referring medical professional as deemed necessary for my medical treatment.
Personal and medical information is confidential and will only be disclosed to third parties with client permission.
Information is collected and used under the Health Information Act.
Privacy and Sharing of Information Policy
In order to best serve our clients, FSA requires 24 hours notice for any cancellations or changes to the clients’ healthcare appointments. Clients who provide less than 24 hours notice or miss their appointment will be charged a cancellation fee to the credit card on file.
If the healthcare professional misses your appointment (due to tardiness, overbooking, etc.), the client must have FSA call the clinic at the time of the missed appointment to confirm with the reception that the appointment was missed by no fault of your own. In this case, FSA will not charge the client for this missed appointment time.
Cancellation Policy
Flying Start Advocates advocates, documents, and clarifies any confusion that occurs during meetings with clients and healthcare professionals. FSA does not provide direct medical advice. If you require direct medical advice, please speak with your specialized healthcare professional.
No Direct Medical Advice Policy
All clients’ healthcare and personal information will be stored on Jane App. Clients will have access to Jane App for notes provided by the Registered Nurse at Flying Start Advocates and the clients can give Jane App access to their family and support people as they see necessary and safe to do.
It is the client’s responsibility to ensure that the device they are using to bring FSA to their healthcare appointments must have a charged battery. If the device loses its battery charge before or during the appointment, the client will be charged the full appointment fee by FSA.
FSA is not responsible for poor internet connections in other healthcare professional offices. Clients are aware that if the internet connection is too poor, they can use the call feature of their device to have FSA with them during appointments.
Virtual Technology Policy
Flying Start Advocates are committed to providing a safe, respectful, and secure environment for all staff and clients. FSA will not tolerate any words or actions that make others feel threatened or demeaned.
This includes:
Yelling or abusive language
Disrespectful, discriminatory, or demeaning comments
Bullying or intimidation
Sexual Harassment
Threats of violence or threatening behavior
Clients displaying any of these behaviors or behavior deemed inappropriate by FSA Management will have their appointments ended immediately and may be dismissed as clients by our practice.
Zero Tolerance for Abuse Policy
Flying Start Advocates is licensed to practice in English. No other languages will be spoken and no translation services will be provided.
